These results underscore the rationale for the implementation and enforcement of public health policies tailored to young adults. A 2018 Truth Initiative survey found that mint was among the top three favorite flavors among young JUUL users aged 12-24, meaning they chose it last time they vaped. New research shows that mint and menthol e-cigarette use among high school users rose from 16% in 2016 to 57.3% in 2019. Among high school JUUL users, 67.5% reported that their preferred flavor was mint or menthol. Another study, conducted before JUUL pulled its other flavors from the market, found that mint was one of the most popular flavors among high school student JUUL users, but that menthol was less so.

Since 2016, the FDA regulates the flavours used in the e-cigarette market and has recently published an enforcement policy on unauthorised flavours, including fruit and mint flavours, which are more appealing to young users [77]. However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5].

They have to give us all the chemistry and toxicology data, and they have to identify what’s in the product, including the liquid and the aerosol it generates. We can also do physical inspections of their manufacturing facilities, or remote records inspections, to verify the information in an application. We were prepared both operationally and from a scientific review standpoint.
In fact, the overall evidence points to e-cigarettes actually helping people to give up smoking tobacco. There is no evidence that e-cigarettes are undermining England’s falling smoking rates. Instead the evidence consistently finds that e-cigarettes are another tool for stopping smoking and in my view smokers should try vaping and vapers should stop smoking entirely. Emerging evidence suggests some of the highest successful quit rates are now seen among smokers who use an e-cigarette and also receive additional support from their local stop smoking services. Marijuana-containing vape products contain concentrated forms of the plant, which means THC levels are many times higher than in the highest-quality marijuana.
Well, if they have, it’s not public because that’s not a public process. But one can look around and see that no e-cigarettes have been approved for use as safe and effective cessation aids. Ultimately, the legal burden is on the companies to demonstrate that the benefit to adults is going to outweigh the harm to kids.
Between 2011 and 2018, past-30-day e-cigarette use grew dramatically among middle school (grades 6-8) and high school (grades 9-12) students. E-cigarettes have been the most commonly used tobacco product by youth in the United States since 2014. Dual use, or use of e-cigarettes and conventional cigarettes by the same person, is common among youth and young adults (ages 18-24). According to the CDC, 15% of EVALI patients were less than 18 years old. Brain development begins during the growth of the fetus in the womb and continues through childhood and to about age 25. Nicotine exposure during adolescence and young adulthood can cause addiction and harm the developing brain. Equally disturbing are two reported deaths and thousands who have been injured or burned because of “vape pen” explosions.
PHE’s remit letter for 2014 to 2015 requested an update of the evidence around e-cigarettes. PHE commissioned Professors Ann McNeill and Peter Hajek to review the available evidence. The review builds on previous evidence summaries published by PHE in 2014. All of our local NHS Stop Smoking Services now proactively welcome anyone who wants to use these devices as part of their quit attempt and increase their chance of success. Cancer Research UK is funding more research to deal with the unanswered questions around these products including the longer-term impact.
The review process is called “premarket evaluation.” A federal judge ordered the FDA to complete its process by September 2021. On the deadline last year, the FDA said it had ruled on 93% of the products, but needed more time to decide on Juul and other major applicants. In October 2021, the first product to receive an “okay” from the FDA was R.J. Reynolds’ Vuse Solo refillable device and corresponding tobacco-flavored pods.
The evidence for e-cigarettes as a tool to help adults quit smoking is limited. To date, no e-cigarettes have been approved by the FDA as smoking-cessation devices. In 2019, more than one in six (15.2%) New York City high school students reported using e-cigarettes.
Scientists are still learning about the long-term health effects of e-cigarettes. CDC continues to recommend that people should not use e-cigarette, or vaping, products that contain THC, particularly from informal sources like friends, or family, or in-person or online dealers. In summary, it seems that either smoking or nicotine vaping may adversely impact on COVID-19 outcome.
But is smoking e-cigarettes (also called vaping) better for you than using tobacco products? Michael Blaha, M.D., M.P.H., director of clinical research at the Johns Hopkins Ciccarone Center for the Prevention of Heart Disease, shares health information about vaping. The large increase in the percent of respondents using non-flavored instead of flavored e-cigarettes after the bans could produce public health gains, as added flavors can increase the toxicity of inhaled e-cigarette aerosols (24, 25). In addition, further research is needed to explore whether allowing only non-flavored e-cigarettes might be more beneficial for public health than also allowing tobacco-flavored versions. This is especially important if other measures are implemented to push smokers away from smoked tobacco products or to attract smokers to e-cigarettes, making using flavors to attract smokers less necessary or unnecessary at all.
That’s not to say that e-cigarettes haven’t helped some people transition away from conventional cigarettes. In the meantime, Dr. Baldassarri says population rates of smoking and vaping should be closely monitored to ensure that the regulatory steps taken by the FDA are optimizing protection of public health. “We want to see cigarette smoking rates decline, and we want vaping to decline among youth and non-smokers,” he says. A pharmacist in China created what would be the first commercially successful electronic cigarette in 2003. “He wanted to develop a pure form of nicotine that could help smokers quit, but without the thousands of harmful chemicals that come from burning cigarettes,” explains Suchitra Krishnan-Sarin, PhD, co-director of the Yale Tobacco Center for Regulatory Science. Finally, the report provides detailed information on advertising and promotional expenditures by domestic e-cigarettes manufacturers between 2015 and 2018. The Commission found that spending on advertising and promotion more than tripled in three years, from $197.8 million in 2015 to $643.6 million in 2018. Previous studies have shown that a majority of people using e-cigarettes want to quit. But it’s been unclear whether products used to stop smoking traditional combustible cigarettes would also work for e-cigarettes.
And to fast forward to today, we have taken final action, one way or the other, on more than 6.6 million of the almost 6.7 million products that were the subject of timely submitted applications. Roy S. Herbst, MD, PhD, deputy director for clinical affairs and chief of medical oncology at Yale Cancer Center and Smilow Cancer Hospital, says he would like to see more clinical trials on this subject. “Ideally, we would encourage someone who is already smoking to switch to e-cigarettes,” Dr. Herbst says. The order only applied to pods that go in refillable cartridge-based devices, and not to disposable e-cigarettes. Unlike e-cigarette devices that come with a spot to place a cartridge, disposable e-cigarettes contain everything in one unit. The study, published on May 23, 2022, in Tobacco Control, is the first to look at the health care costs of e-cigarette use among adults 18 and older.
The difference in opinion worldwide may be due to different restrictions imposed. For example, while no more than 20 ng/mL of nicotine is allowed in the EU, e-liquids with 59 mg/dL are currently available in the United States. Nevertheless, despite the national restrictions, users can easily access foreign or even counterfeit products online. Electronic nicotine dispensing systems (ENDS), commonly known as electronic cigarettes or e-cigarettes, have been popularly considered a less harmful alternative to conventional cigarette smoking since they first appeared on the market more than a decade ago. E-cigarettes are electronic devices, essentially consisting of a cartridge, filled with an e-liquid, a heating element/atomiser necessary to heat the e-liquid to create a vapour that can be inhaled through a mouthpiece, and a rechargeable battery (Fig. 1) [1, 2]. Both the electronic devices and the different e-liquids are easily available in shops or online stores.
In Texas, ABC13 has reported e-cigarette use among elementary school students, with leading experts in the field explaining the severity of the trend. A very young company, Elf Bar entered the vape pens industry with extreme confidence in 2018 and immediately gained the attention of a whole new audience of smokers who up until then had been put off by the more hands-on approach vaping ordinarily requires.
In 2014, 50% of Alaska Native adults and 21% of non-Native adults used tobacco products, the report said. By 2021, the rate for Native adults fell to 43% while the rage for non-Native adults was unchanged over that period. While a quarter of all Alaska adults reported that they used tobacco products in 2021, different patterns are emerging according to socioeconomic status, geography and other factors, the new report said. We believe this is an important step in establishing an in vitro system by which to investigate the effects of e-cigarettes on the airways. Finally, we have not assessed the effects of flavours on cellular effects,43 partly because this adds another layer of uncertainty and potential confounders. However, the data provide a background on which to study these other potential factors, with and without nicotine.
That figure is controversial and might be a little high, says Kenneth Warner, a tobacco policy researcher at the University of Michigan. But, he adds, “The worst critics of e-cigarettes would probably argue they’re a half to two-thirds less dangerous. But from a practical view, they’re probably on the order of 80% to 85% less dangerous, at least.” Public health experts and tobacco researchers are trying to find out. But the science clearly indicates vaping is not a safe or healthy alternative to smoking. We’ll continue to support research into the health consequences of this and other tobacco product trends that aim to appeal to a new generation of users. New York State has enacted strong policies to protect youth from accessing tobacco products and denormalize tobacco and nicotine use to prevent lifelong addiction, illness, and early death. Policies include banning the sale of flavored vapor products, ending price discounts on tobacco and vapor products, and banning the exterior display of tobacco and vapor product ads near schools.
Users have reported a growing list of potential e-cigarette side effects such as seizures, strokes, respiratory failure and bronchiolitis obliterans organizing pneumonia, or BOOP. The AMA promotes the art and science of medicine and the betterment of public health. The AMA has been a leading force to eliminate public smoking and warn people about the dangers of smoking, including secondhand smoke, since the 1960s.

The Sonoma County Board of Supervisors today voted to ban the sale of flavored tobacco products and e-cigarettes in unincorporated areas of the county. The changes to the county’s tobacco retail ordinance are in response to public health data that show e-cigarettes and flavored tobacco use increasing among high school-aged youth. Yet, they contain many substances beyond nicotine, including propylene glycol, glycerin, flavorings and potentially harmful chemicals such as formaldehyde and metals, which could pose significant health risks such as respiratory disease, cardiovascular disease and cancer. For example, in an analysis using primary human umbilical vein endothelial cells (HUVEC) exposed to 11 commercially-available vapours, 5 were found to be acutely cytotoxic, and only 3 of those contained nicotine [24]. In addition, 5 of the 11 vapours tested (including 4 that were cytotoxic) reduced HUVEC proliferation and one of them increased the production of intracellular reactive oxygen species (ROS) [24]. The most recent evidence suggests that e-cigarettes may pose their own unique health harms and that comparison to cigarettes may not be the only relevant question for determining their impact on individual health. Nicotine is the primary agent in regular cigarettes and e-cigarettes, and it is highly addictive. It causes you to crave a smoke and suffer withdrawal symptoms if you ignore the craving. First, it relied on self-reported data, which introduces the potential for misclassification or recall bias. In addition, social desirability and recall bias may have resulted in underreporting of both e-cigarette use and smoking status. It is important to note that these data provide a snapshot of e-cigarette use specifically in 2021, and assessing the overall impact of the entire COVID-19 pandemic on e-cigarette use presents challenges.
A study in the Journal of the American Medical Association in 2015 found a connection too. Researchers surveyed 2,500 Los Angeles high school students who had never smoked. They found that kids who used e-cigs were more likely than non-users to smoke cigarettes or other tobacco products over the next year.
In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects. It would seem that future studies should analyse the possible toxic effects of humectants and related products at concentrations similar to those that e-cigarette vapers are exposed to reach conclusive results. It is well known that nicotine is extremely addictive and has a multitude of harmful effects.
“Our finding indicates that health care expenditures for a person who uses e-cigarettes are $2,024 more per year than for a person who doesn’t use any tobacco products,” said lead author Yingning Wang, PhD, of the UCSF Institute for Health & Aging. Use of electronic cigarettes costs the United States $15 billion annually in health care expenditures – more than $2,000 per person a year – according to a study by researchers at the UC San Francisco School of Nursing. “Electronic Cigarette” means any device that can provide an inhaled dose of nicotine by delivering a vaporized solution [and] includes the components of an electronic cigarette including, but not limited to, liquid nicotine. The FDA has never approved an e-cigarette as a smoking cessation device. Studies into whether the devices help people quit smoking have yielded mixed results. When Juul first hit the market in 2015, its vape liquid contained much higher levels of nicotine than e-cigs that had already been on the market. Each Juul pod contains as much nicotine as 20 regular cigarettes, according to the Centers for Disease Control and Prevention.

E-cigarettes typically operate on a rechargeable lithium-ion battery.
Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]). In contrast, other studies found acute microvascular endothelial dysfunction, increased oxidative stress and arterial stiffness in smokers after exposure to e-cigarettes with nicotine, but not after e-cigarettes without nicotine (reviewed in [20]). In women smokers, a study found a significant difference in stiffness after smoking just one tobacco cigarette, but not after use of e-cigarettes (reviewed in [20]). One of the first studies in humans involved the analysis of 9 volunteers that consumed e-cigarettes, with or without nicotine, in a ventilated room for 2 h [8].
