Some look like a regular cigarette, but many resemble everyday products like pens, USB drives, highlighting markers, or colorful toy-like items. E-cigarettes are not safe for youth, young adults, and pregnant women, and can be dangerous for adults who use tobacco products. Electronic cigarettes, or e-cigarettes, are devices that use a heating element to turn flavored liquids into an aerosol that the user inhales. Most of these devices are used to deliver nicotine to the user, which is why another name for them are ENDS (electronic nicotine delivery systems). E-cigarette devices come in a wide variety of shapes, sizes, and styles.

With what’s known about it right now, though, experts say it isn’t safe—or even safer— to vape marijuana. This discussion covered the topic of e-cigarette use and its potential treatment with cytisinicline, a plant-based medication which may aid vapers in quitting. Today, experts admit they still don’t fully know what e-cigarette vapor actually contains. Previous laboratory studies have, for instance, found that tobacco or mint flavors contain more toxic metals than sweet ones. There were 134 reports of e-cigarette batteries overheating, catching fire, or exploding between 2009 and January 2016, according to Michael Felberbaum, an FDA spokesperson. The new rules will allow FDA to review the safety of batteries and eventually take action to protect the public.

For use frequency pre-ban, respondents reported whether they used e-cigarettes daily (every day or most days in a week ) or weekly (at least once a week, but not most days). For post-ban, respondents reported whether they used e-cigarettes daily, weekly, less than weekly, or not at all. For both pre- and post-ban, respondents reported all e-cigarette flavors they used, the flavor that they primarily used, and the different ways they obtained e-cigarettes. The Department has sent a statewide health alert to health care providers and local health departments following recent reports of severe lung disease in people who have used vaping products. The cytotoxic and pro-inflammatory effects of different e-cigarette flavouring chemicals were also tested on two human monocytic cell lines—mono mac 6 (MM6) and U937 [86]. Among the flavouring chemicals tested, CAD was found to be the most toxic and O-vanillin and pentanedione also showed significant cytotoxicity; by contrast, acetoin, diacetyl, maltol, and coumarin did not show any toxicity at the concentrations assayed (10–1000 µM). Of interest, a higher toxicity was evident when combinations of different flavours or mixed equal proportions of e-liquids from 10 differently flavoured e-liquids were tested, suggesting that vaping a single flavour is less toxic than inhaling mixed flavours [86]. Also, all the tested flavours produced significant levels of ROS in a cell-free ROS production assay.

As smoking has declined dramatically in the U.S., vaping has gained tremendous popularity very quickly. A 2018 study reported 20% of high schoolers vaped at least once in the past 30 days. Federal regulators have targeted flavored vape liquids to rein in underage sales and use. Early results from a national survey of teen tobacco and e-cig use show vaping among teens continued to rise well into 2019. The National Institute on Drug Abuse found 1 in 4 high school seniors had vaped nicotine products within the last month. For 10th graders, the rate was 1 in 5, and for 8th graders, it was 1 in 11.

As of April 1st, 2024, 1,061 municipalities, plus 26 states, commonwealths, and territories restrict e-cigarette use in 100% smokefree venues. Statistical analyses were performed using GraphPad PRISM 6.0 software package (San Diego, California, USA). Results are expressed as the median with IQR, unless specified otherwise. Because e-cigarettes contain nicotine derived from tobacco, they are now subject to government regulation as tobacco products. In December 2019, the federal government raised the legal minimum age of sale of tobacco products from 18 to 21 years, and in January 2020, the FDA issued a policy on the sale of flavored vaping cartridges. Products marketed for therapeutic purposes (for example, marketed as a product to help people quit smoking) are regulated by FDA’s Center for Drug Evaluation and Research (CDER). FDA published a rule clarifying when products made or derived from tobacco are regulated as tobacco products, drugs, and/ or devices.

More adults 18 to 24 years old used e-cigarettes (11.0%, or roughly 3.4 million people) compared with adults 25 to 44 (6.5%) or 45 and older (2.0%). In 2022, 2.55 million U.S. middle and high school students currently used e-cigarettes. In 2018, the rapid rise of tobacco products led the U.S. surgeon general to issue an advisory about the youth e-cigarette “epidemic.” In 2019, youth e-cigarette use increased to even higher levels. The devices, also known as vapes, can cause serious health implications, including contributing to detrimental inhalation patterns, and heightened risks for youth’s developing brains, according to medical researchers. Because of the popularity of certain flavored e-cigarette products among children, FDA stated in January 2020 that it would prioritize enforcement actions against the manufacture and sale of most flavors in cartridge-based e-cigarettes. However, the data provide a background on which to study these other potential factors, with and without nicotine.

Finally, diacetyl, pentanedione, O-vanillin, maltol, coumarin, and CAD induced significant IL-8 secretion from MM6 and U937 monocytes [86]. It should be borne in mind, however, that the concentrations assayed were in the supra-physiological range and it is likely that, once inhaled, these concentrations are not reached in the airway space. Indeed, one of the limitations of the study was that human cells are not exposed to e-liquids per se, but rather to the aerosols where the concentrations are lower [86].

Then there’s tobacco-flavored e-cigarettes, and in that case the benefit–harm equation is different, because tobacco-flavored e-cigarettes are not nearly as popular with kids as flavored e-cigarettes. And so that’s how we have been able to authorize some tobacco-flavored e-cigarettes for marketing. This is because under federal law, although the FDA has the power to regulate the tobacco industry, they can do so with certain limitations. For example, nicotine and cigarette products cannot be banned, but flavorings such as mint (sold as menthol) and fruit can.
