That’s the key finding of my team’s 2023 study, published in JAMA Network Open. Initially marketed as a safer alternative to traditional cigarettes for adults looking to quit smoking, these products quickly caught the attention of younger demographics. A survey of more than 4,000 New Jersey adolescents found that 25% were vaping — far outpacing cigarette smoking, according to a 2020 study.
Still, it’s far from safe and can lead to nicotine addiction and potentially fatal lung disease. Electronic cigarettes—or e-cigarettes—are battery-powered devices that convert a liquid, usually with nicotine, into an inhalable aerosol. They are designed as an alternative to smoking traditional cigarettes. First introduced in the United States about 15 years ago, e-cigarettes have evolved from disposable, single-use products to more sophisticated devices that can be refilled and customized (fig. 1). One of the largest studies done on this was published in The New England Journal of Medicine in 2019. The study, which included 886 randomized participants, compared e-cigarettes and nicotine replacement therapy, which included patches, gum, lozenges digiflavor helix, nasal spray, and mouth spray.
Never smokers and former smokers were more likely to quit e-cigarettes. While there were no significant differences in quitting all e-cigarette use after each of the three state bans, e-cigarette users in New York were less likely to use banned flavors after the ban than those in New Jersey. The use of menthol flavor was also higher among smokers than former and never smokers. The percent of non-TM flavors had a reverse pattern, being lowest among daily smokers (46.7%), higher progressively along with those smoked weekly or less, former smokers, and highest among never smokers (79.1%). After the ban, the percentages of e-cigarette users who primarily used tobacco, menthol, and non-TM flavors decreased generally except the menthol share among never smokers did not change. After the ban, the percentage of those who quit using e-cigarettes was highest among never smokers (23.1%) and former smokers (13.5%) compared with 3.2% and 7.8% among those who smoked daily and weekly, respectively.
Continuing to monitor flavored tobacco product sales is key to assessing policies like these and informing further policy development and implementation. Food and Drug Administration’s 2009 ban on flavored cigarettes reduced adolescent tobacco use. Validated 7-day point prevalence abstinence rates were affected less, and effect sizes for all outcomes were close to those predicted, but the lower quit rates have reduced the statistical power for the primary outcome. Another key limitation of open-label trials is that participants’ expectations can affect outcomes. In smoking cessation studies, the results can be biased if participants randomized to what they perceive as an inferior option are less likely to use their treatment or are more likely to drop out of the trial.

Then there are other pouches that contain synthetically made nicotine, which is made in the lab and is not derived from tobacco. I don’t know if it’s always apparent from the label whether it’s synthetic or not, but that’s a nuance that we’ve seen in our research. This trend was exacerbated by aggressive marketing, which often targeted younger audiences with social media campaigns and appealing flavors, leading to widespread use among teens throughout the decade.

However, the various flavors and flashy marketing campaigns attracted the attention of teenagers, public health experts say. Non-combustible tobacco products, such as e-cigarettes, generally have lower health risks to the user than combusted tobacco products. The US National Academies of Sciences, Engineering, and Medicine (NASEM) found conclusive evidence that switching completely from cigarettes to e-cigarettes reduces a person’s exposure to many toxicants and carcinogens that are present in combusted tobacco cigarettes1. E-cigarettes have become the most commonly used tobacco product among U.S. and Iowa youth, and their popularity has risen dramatically over the past several years. According to the Iowa Youth Survey (IYS), Iowa 11th-graders were far more likely to use e-cigarettes compared to traditional cigarettes.
The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases. This study examined the impact on adult e-cigarette users from the recent e-cigarette-only flavor bans in Washington State, New Jersey, and New York. We found that most respondents continued to use e-cigarettes with banned flavors post-ban and obtain banned-flavor e-cigarettes through legal channels. Enforcing compliance of retailers to the policy is crucial to control e-cigarette use. Because most tobacco use starts during adolescence, actions to protect our nation’s young people from a lifetime of nicotine addiction are critical. Previous trials that compared ECs and NRT mostly complemented these treatments with intensive behavioral support.5 This raises an important question of whether ECs are effective without such clinical involvement. The present trial suggests that they are, but it does not provide a definitive answer. Although only minimal behavioral support was included, smokers were still asked to set up a TQD and their smoking status was checked monthly, features that are not available to smokers using ECs on their own. To see whether public health messages on EC use for smoking cessation need to include advice to use any additional support, further studies are needed that compare effects of different levels of behavioral support added to ECs. Adults who report puffing e-cigarettes, or vaping, are significantly more likely to have a heart attack, coronary artery disease and depression compared with those who don’t use them or any tobacco products, according to the study findings. The vapor produced by e-cigarettes usually contains nicotine, heavy metals, volatile organic compounds, carcinogens (cancer causing chemicals), and in some cases, flavoring linked to serious lung disease. These and other harmful substances present in e-cigarette vapor can negatively affect the respiratory health of users. E-cigarettes are the most popular tobacco product among youth, with about one in five high school students using e-cigarettes in 2020. Youth and young adults are widely exposed to e-cigarette marketing and have high awareness of the products. Among middle and high school students who reported contact with a potential source of tobacco advertising in 2019, such as going to a convenience store or gas station, watching television, or reading magazines, nearly 70% (69.3%) were exposed to e-cigarette marketing.
We believe this is an important step in establishing an in vitro system by which to investigate the effects of e-cigarettes on the airways. Finally, we have not assessed the effects of flavours on cellular effects,43 partly because this adds another layer of uncertainty and potential confounders. However, the data provide a background on which to study these other potential factors, with and without nicotine.
And finally, the exposure to point-of-sale marketing of e-cigarette has also been identified to affect the smoking cessation success [96]. Is there sufficient toxicological data on all the components employed in e-liquids? Do we really know the composition of the inhaled vapour during the heating process and its impact on health?
The ingredients, including nicotine levels, vary between brands and flavors, but many products are not thoroughly or accurately labeled. From introducing appealing flavors to offering college scholarships, manufacturers and sellers of e-cigarettes aggressively target young people. There are few federal restrictions on e-cigarette marketing, allowing companies to promote their products through traditional outlets — such as TV and radio — despite a ban in 1971 on cigarette advertising in both outlets to reduce cigarette marketing to children. E-cigarette companies also take advantage of other marketing outlets, including the internet, retail environments, and recreational venues and events. Nicotine is an addictive substance, but its level of addictiveness can vary substantially depending on its mode of delivery. Nicotine delivered by the combustion of tobacco is the most addictive form.12 The rise in popularity of e-cigarettes that can deliver levels of nicotine similar to combustible cigarettes is causing concern about the potential risk for addiction.
The most recent data from the CDC’s 2019 National Youth Tobacco Survey confirms JUUL’s grip on America’s kids. More than 59% of high school e-cigarette users reported that JUUL was their “usual brand.” Among middle school e-cigarette users, the number was more than 54%. This equates to more than 3 million children reporting regular use of JUUL. Investigative reporting by Reuters shows that even early in its operations, JUUL was aware that its products were attracting unprecedented youth use, yet, in the face of enormous revenue growth, the company failed to take effective action to stem that trends. This work is part of a growing body of research on the potential toxicity and health impacts of e-cigarettes reported by the American Heart Association Tobacco Regulation and Addiction Center, for which UofL serves as the flagship institute. In the study, published in Nicotine and Tobacco Research, researchers compared heart rate and heart rate variability in mice exposed to vape aerosols containing different types of nicotine.
No matter how it’s delivered, nicotine is harmful for youth and young adults. E-cigarettes typically contain nicotine as well as other chemicals that are known to damage health. Compared with other race/ethnicity groups, Hispanics were the least likely to quit and Blacks were the most likely to continue using banned flavors after the ban.
