With the rise in electronic cigarettes (e-cigarette, e-cigs or vapes) and vaping use among youth, teens and young adults, the Health Department and our team are working to prevent initiation and reduce vaping of all substances in Vermont. The researchers also raise concerns about risks of short- as well as long-term use of EVPs. Since the end of 2019, it is illegal to sell vaping products to people younger than 21 years. Current evidence indicates that using e-cigarettes is dangerous, especially for young people and people who have never smoked. A 2016 study found that people who use or have used e-cigarettes are less likely to stop smoking altogether.
The lockdowns made the collection of CO readings difficult, particularly during 2 periods in 2022 shown in eFigure 2 in Supplement 2 when most study dropouts were recorded. This reduced the validated quit rates, but all 3 study arms were affected equally. This cluster of events is the likely reason for quit rates being lower than expected in our power calculations.
Youth and young adults had more than three times the odds of using fruit-flavored e-cigarettes compared to older adults. A 2017 study by Truth Initiative found 25% of survey respondents, aged 15-24, recognized a JUUL e-cigarette device when shown a photo of the product. And among those who recognized JUUL, 25% reported that use of this product is called “JUULing,” indicating that this product is so distinctive, it’s perceived as its own category. However, it’s likely that recognition is higher now given that JUUL’s sales market share continued to rise throughout 2018 and top out at 75% among U.S. e-cigarette sales in July 2019. Scientists are still learning about the long-term health effects of e-cigarettes. In a document issued Monday, Chinh said the rate of smokers has dropped by 0.5% on average every year among males. The Food and Drug Administration estimates that more than 3.5 million middle and high school students used e-cigarettes in 2018, though sales to minors are prohibited.

The research lacks a control group of teens who did not vape at all, but the pattern evident within a US sample of 200 e-cigarette users who avoided cigarettes is still concerning. Finally, the researchers compared the levels of nicotine in the blood serum of people after they had vaped e-cigarettes with the levels in people who smoked traditional cigarettes. The flavoring liquid for electronic cigarettes, or e-cigarettes, may increase the risk of cardiovascular disease when inhaled, according to a study led by researchers at the School of Medicine. Public health groups want the FDA to more aggressively police regular cigarettes and flavored e-cigarettes that appeal to teenagers.
For example, some Juul pods contain more nicotine than many other e-cigarettes. Research is mixed on whether vaping is safer than smoking, and e-cigs like Juul are so new that not a lot of research has been done. In November 2018, the FDA announced restrictions on the sale of flavored e-cigarette vape fluids. In September 2019, the Wall Street Journal reported that the FDA had opened a criminal investigation into Juul Labs to determine whether the company used deceptive advertising and targeted minors.
The FDA said the company marketed its products as “modified risk tobacco products” without FDA clearance. The agency stressed that there was no scientific proof that Juul products posed less risk or were less harmful than tobacco products. Flavored vape fluids have been criticized as a marketing ploy to lure teens and young adults to vaping.
Clearly the dangers extend beyond what is being done to the lungs and heart. Adults who used e-cigarettes at least once a week before the flavor bans were recruited online. Respondents reported their e-cigarette use, primarily used flavor, and ways of obtaining e-cigarettes before and after the bans.
Extended EC use may be beneficial for some previous smokers by helping them to maintain some of the subjective rewards of smoking, avoid postcessation weight gain, or prevent relapse. However, although EC use is expected to pose few health risks of smoking, some adverse health outcomes of long-term EC use are likely.25,26 Varenicline is not used long term and so has an advantage in this respect. Participants were included if they smoked at least 10 cigarettes per day for at least 5 years, had expired air carbon monoxide (CO) reading of 9 parts per million (ppm) or greater, were age 25 to 45 years, and were motivated to stop smoking. Instead of bathing lung tissue with a therapeutic mist, just as a nebulizer does, vaping coats lungs with potentially harmful chemicals. E-liquid concoctions usually include some mix of flavorings, aromatic additives and nicotine or THC (the chemical in marijuana that causes psychological effects), dissolved in an oily liquid base. “We think that some of the vaporized elements of the oil are getting deep down into the lungs and causing an inflammatory response,” explains Broderick.

As of April 1, 2024, 1,061 municipalities, plus 26 states, commonwealths, and territories restrict e-cigarette use in 100% smokefree venues. As the user draws on the device, the battery heats the e-liquid to produce aerosol (not water vapor). The battery-operated devices come in many forms and can look like conventional cigarettes, pens or even sleek tech gadgets. This way of taking in nicotine poses health risks to both users and non-users. The 2021 report also provides details on some characteristics of e-cigarette products, including flavors and nicotine concentration, as well as the bundling of the components in cartridge systems. The data shows that in 2021, 69.2 percent of e-cigarette cartridges either sold or given away contained menthol-flavored e-liquids, and the rest were tobacco-flavored. The monitoring of other tobacco product sales in conjunction with the monitoring of e-cigarette sales is necessary to provide a comprehensive picture of the overall U.S. tobacco product market.
The concentration of nicotine in liquid products is higher than most other tobacco products. With millions of teens and young adults using e-cigarettes, a new generation could become addicted to nicotine and face other health risks. For some adult smokers, e-cigarettes may offer a less harmful substitute for traditional cigarettes. However, long-term effects on users, second-hand exposure, and environmental effects are not yet fully known.
The Ministry of National Defense and the Ministry of Public Security are instructed to ramp up patrols at the border, and to take down e-cigarette suppliers. The Ministry of Industry and Trade needs to manage the domestic market and fine individuals and organizations that sell e-cigarettes. Among those the team could identify were several potentially harmful substances, including caffeine, three chemicals never previously found in e-cigarettes, a pesticide and two flavorings linked with possible toxic effects and respiratory irritation. To address this need, we used the 2021 BRFSS to examine recent patterns of e-cigarette use among US adults.
Although there’s no definitive answer at this point, experts do have a theory about how vaping harms lungs. Second, we reported the distribution of the primarily used e-cigarette flavor before and after the ban with paired t-tests to evaluate the differences. Third, a multinomial logistic regression (PROC LOGISTIC in SAS, version 9.4) was used to estimate the associations between demographics, e-cigarettes and smoking preference and use status, and the flavor that respondents primarily used after the ban. Fourth, we reported the distribution of ways of obtaining e-cigarettes before and after the flavor ban. Finally, the use of non-e-cigarettes tobacco products after the ban among different categories of respondents was assessed. Respondents were also asked whether they were aware of the ban, supported the ban, and extent to which they perceived local retailers were compliant with the ban. Over the next few years, the optimism over e-cigarettes waned as their popularity skyrocketed.

Enter e-cigarettes, which were new, high-tech, and came with no proven health risks. There were no long-term studies yet, but common sense dictated that if you wanted to quit inhaling tobacco through smoking, the least you could do was switch to e-cigarettes. To date, the FDA has authorized only 23 tobacco-flavored e-cigarette products and devices. These are the only vape products legally marketed and sold in the U.S. We do know from my research that they can cause acute adverse events. Some people have reported developing mouth lesions from using them too much.
To address this problem, the Campaign for Tobacco-Free Kids and other public health organizations have called on the U.S. Food and Drug Administration to eliminate all flavored e-cigarettes. While FDA has made progress in reviewing marketing applications for flavored e-cigarettes, it has repeatedly missed deadlines to complete its review of major products and failed to clear the market of these illegal flavored products. The researchers concluded that, if these results are confirmed in humans, regulating nicotine salts through minimum pH standards or limits on acid additives in e-liquids may mitigate the public health risks of vaping. E-cigarettes are not currently approved by the FDA as a quit-smoking aid.
About one-quarter of U.S. youth and young adults have ever tried e-cigarettes. While vaping might help you quit smoking, it probably won’t help you quit nicotine altogether. Some damage to your lungs from vaping can heal or get better with medications. Over time, constant irritation to your lungs can lead to health problems (like asthma and COPD) that won’t go away. ENDS may be manufactured to look like conventional combusted cigarettes, cigars, or pipes. Larger devices, such as tank systems or mods, bear little or no resemblance to cigarettes.
Compared with other race/ethnicity groups, Hispanics were the least likely to quit and Blacks were the most likely to continue using banned flavors after the ban. Respondents were both less likely to quit e-cigarette use and more likely to continue using banned flavors if they had used e-cigarettes for a greater amount of time or had weaker intentions to quit before the ban. Respondents who used e-cigarettes because of the flavor were more likely to continue using banned flavors (statistically significant for non-TM flavors but not for menthol). Those primarily using non-TM flavors before the ban were most likely to quit using e-cigarettes and more likely to continue the same flavor afterward.
And to fast forward to today, we have taken final action, one way or the other, on more than 6.6 million of the almost 6.7 million products that were the subject of timely submitted applications. Roy S. Herbst, MD, PhD, deputy director for clinical affairs and chief of medical oncology at Yale Cancer Center and Smilow Cancer Hospital, says he would like to see more clinical trials on this subject. “Ideally, we would encourage someone who is already smoking to switch to e-cigarettes,” Dr. Herbst says. The order only applied to pods that go in refillable cartridge-based devices, and not to disposable e-cigarettes. In recognition of these and other risks related to e-cigarettes, the U.S. surgeon general issued an advisory on e-cigarette use among youth, urging parents, teachers, health professionals and states to take action to stop the epidemic among youth. Nearly five times as many high school students use e-cigarettes than smoke cigarettes. Tetrahydrocannabinol (THC)-containing e-cigarette or vaping products was linked to most EVALI cases. Emergency department visits related to e-cigarette, or vaping, products have sharply declined in recent years, but the CDC continues to monitor reports.
Another notable observation was the high proportion of daily use among persons who reported current e-cigarette use, indicating a possible transition toward established use and potential nicotine dependence. These findings are of value to the tobacco regulatory science community and to policy makers, and they underscore the rationale for the implementation and enforcement of public health policies tailored to young adults. We observed a high proportion (71.5%) of individuals aged 18 to 20 years who reported current e-cigarette use without concurrent use of combustible cigarettes.
