The 2020 Surgeon General’s Report on smoking cessation found that there is “inadequate evidence” to conclude that e-cigarettes increase smoking cessation.
Urgent measures are necessary to prevent uptake of e-cigarettes and counter nicotine addiction alongside a comprehensive approach to tobacco control, and in light of national circumstances. While nicotine is the addictive substance in cigarettes, most of the harm from smoking comes from the thousands of other chemicals in tobacco smoke, many of which are toxic. Under the PACT Act, delivery sales of electronic nicotine delivery systems must also comply with state, local or tribal laws involving excise taxes on these devices. Further, in 2018, one in 15 (6.7%) middle school students reported using e-cigarettes.
The lawsuits claim the company marketed nicotine products to minors and Juul devices led to serious health problems. Lawyers expect people to file more Juul lawsuits in the litigation as time passes. The trendy e-cigs appeal to young adults and teens who often don’t realize Juul products are e-cigarettes or contain nicotine. Public health researchers have found teens using the term “juuling” instead of “vaping” to distinguish Juul from other e-cigs even though the two activities are the same. E-cigarettes deliver a liquid to users as a vapor instead of a harsher smoke.
Electronic cigarettes produce an aerosol by heating a liquid that usually contains nicotine, flavorings and other chemicals. Several states have imposed restrictions on the sale of e-cigarettes. There are also restrictions on liquids with flavors that may be more attractive to younger people. When the user sucks on the mouthpiece, the heating element vaporizes the solution, which the person then “vapes,” or inhales.
These liquid pods also contain some of the same toxic chemicals found in smoke from traditional cigarettes, which is, in a word, poison. When tested, aldehydes, traces of metal, and other carcinogens responsible for playing a role in lung and oral cancers were present. While some adult users may have successfully quit smoking and attribute it to “vaping,” the fact remains that e-cigarettes contain nicotine, a highly addictive, dangerous substance.
Treatment adherence outcomes included attendance at monthly sessions and self-reported use of allocated and nonallocated products. Other outcomes included ratings of treatments, monitoring of adverse reactions and recording of serious adverse events. Although the popularity and use of e-cigarettes continues to increase, there is a lack of data on their potential health effects. Food and Drug Administration (FDA) has issued a proposed rule to include e-cigarettes under its authority to regulate certain tobacco and nicotine-containing products. E-cigarettes have a battery-operated component that heats liquid containing nicotine, flavorings, and other chemicals.

Electronic cigarettes, or e-cigarettes, are part of a category of products called Electronic Smoking Devices (ESDs), which are designed to mimic combustible tobacco cigarettes and, in most cases, contain varying levels of nicotine. Vape products are taxed at the sales tax rate rather than the tobacco or cigarette tax rate. While vape products are not covered by the Smoke-free Air Act, each business can prohibit the use of vape devices indoors, but this is not required by state law. Iowa law places several restrictions on youth access to e-cigarettes, including but not limited to, prohibiting the sale, distribution, possession, purchase and use of vapor products to anyone under the age of 21. And a lot’s being said about vaping and its effects – but there’s also some confusing or conflicting info out there. As of April 1, 2024, 1,061 municipalities, plus 26 states, commonwealths, and territories restrict e-cigarette use in 100% smokefree venues. Experts are questioning whether punishment is an effective deterrent to nicotine use in the first place. How many students have been caught at the Houston area’s largest school districts?
Interestingly, most of these reports linking COVID-19 harmful effects with smoking or vaping, are based on their capability of increasing the expression of angiotensin-converting enzyme 2 (ACE2) in the lung. It is well known that ACE2 is the gate for SARS-CoV-2 entrance to the airways [106] and it is mainly expressed in type 2 alveolar epithelial cells and alveolar macrophages [107]. To date, most of the studies in this field indicate that current smokers have higher expression of ACE2 in the airways (reviewed by [108]) than healthy non-smokers [109, 110]. The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76].
Despite their popularity, little is known about their health effects. Some suggest that e-cigarettes likely confer lower risk compared to combustible tobacco cigarettes, because they do not expose users to toxicants produced through combustion. Proponents of e-cigarette use also tout the potential benefits of e-cigarettes as devices that could help combustible tobacco cigarette smokers to quit and thereby reduce tobacco-related health risks. Others are concerned about the exposure to potentially toxic substances contained in e-cigarette emissions, especially in individuals who have never used tobacco products such as youth and young adults.
E-cigarette use has increased 78 percent in one year for high schoolers and 48 percent in middle schoolers. Additionally, the CDC reports nearly 38 percent of all high schoolers and even 13 percent of middle schoolers have tried “vaping” at least once, and those statistics likely are underreported. Drawn to the fruity flavor cartridges, or trying to wean from traditional tobacco products, or even to curtail hunger, many people believe e-cigarettes or “vaping” is safer than smoking cigarettes and simply not addictive. As shown in Table 1, the majority of the 1624 respondents were males (61.2%), young adults between 25 and 34 years old (56.9%), Whites (59.4%), and had at least a bachelor’s degree (66.8%). The majority of respondents had used e-cigarettes between two and five years (69.6%) and 79.8% had moderate or strong intentions to quit. The percent of respondents saying they used e-cigarettes because of the added flavors or for quitting smoking cigarettes were 54.4% and 36.6%, respectively.
E-cigarettes are not currently available from the NHS on prescription, so you cannot get one from your GP. If you’re pregnant, licensed NRT products such as patches and gum are the recommended option to help you stop smoking. Nicotine replacement therapy (NRT) has been widely used for many years to help people stop smoking and is a safe treatment.
Patients have reported high variability in substances/products they used in vaping, including both tobacco and tetrahydrocannabinol (THC) containing products as well as other products. Many e-cigarettes and vaping materials contain harmful or potentially harmful substances, and it is difficult to know what each individual product contains. The mislabelling of nicotine content in e-liquids has been previously addressed [8, 34]. Of note, several studies have detected nicotine in those e-liquids labelled as nicotine-free [5, 35, 36]. Among the 17 samples tested in this latter study 14 were identified to be counterfeit or suspected counterfeit. A third study detected nicotine in 7 of 10 nicotine-free refills, although the concentrations were lower than those identified in the previous analyses (0.1–15 µg/mL) [5].
Continuous and rigorous implementation of traditional tobacco control measures, such as taxation, clean indoor air policies and public education is also essential. Finally, the recent news of vaping-related illnesses requires swift research and effective guidance to protect the public’s health. In addition to the unknown health effects, early evidence suggests that vaping might serve as an introductory product for preteens and teens who then go on to use other nicotine products, including cigarettes, which are known to cause disease and premature death.
Because of their shape, school teachers might not notice students using JUUL in school, including in classrooms and bathrooms. Only 28% of young people in the U.S. believe e-cigarettes have the potential to cause a lot of harm. Up to 40% of teens who vape are unaware their e-cigarette cartridges contain nicotine. What we do know is that they are not free of toxins and still deliver harmful chemicals. The leading reason for e-cigarette use among adults (25 and older) was the belief that e-cigarettes may be less harmful than combustible cigarettes both to themselves and others, with 79% selecting this as a reason. Additionally, 7 in 10 (71.5%) adults indicated that they believed e-cigarettes could facilitate quitting. This is Quitting, Truth Initiative’s first-of-its-kind, free and anonymous text message quit vaping program for teens and young adults, is helping more than 200,000 young people quit using e-cigarettes. The program first launched in January 2019, created with input from teens, college students and young adults who have attempted to, or successfully, quit e-cigarettes. With the start of a new year, smokers and vapers may have resolved to quit or cut back on the habit to improve their health.
For use frequency pre-ban, respondents reported whether they used e-cigarettes daily (every day or most days in a week ) or weekly (at least once a week, but not most days). For post-ban, respondents reported whether they used e-cigarettes daily, weekly, less than weekly, or not at all. For both pre- and post-ban, respondents reported all e-cigarette flavors they used, the flavor that they primarily used, and the different ways they obtained e-cigarettes. Most respondents continued to use e-cigarettes with banned flavors post-ban. Compliance of local retailers with the ban was not high, and many respondents obtained banned-flavor e-cigarettes through legal channels. However, the significant increase in the use of non-flavored e-cigarettes post-ban suggests that these may serve as a viable alternative among those who used previously used banned or tobacco flavors.
The report also reveals a large increase in the concentration of nicotine in disposable e-cigarettes sold between 2015 and 2018. In 2015, disposable products on average contained 25 mg of nicotine per ml of e-liquid. This concentration increased nearly 60% to 39.5 mg/ml in 2018, increasing their addictiveness. Cartridge-based e-cigarettes contained even higher nicotine concentrations, with the most popular products having concentrations between 51 and 61 mg/ml, while the sale of nicotine-free cartridges was negligible. On the other hand, one potential challenge that the researchers uncovered in the results indicated that people without a cigarette smoking history – in other words, those who have only ever used e-cigarettes – might have a harder time quitting.
Unlike traditional cigarettes, e-cigarettes are made up of a battery-operated heating part — a cartridge (unit) that typically holds nicotine and other chemicals that change into a chemical-filled aerosol when heated. As a result of the FDA’s missed deadlines and inadequate enforcement, flavored e-cigarettes remain widely available online and in stores across the country. Every day flavored e-cigarettes remain on the market, our kids remain at risk. The FDA must act without further delay to remove all flavored e-cigarettes from the market. Despite not authorizing a single synthetic nicotine product, it has allowed these products – including e-cigarettes in kid-friendly flavors – to stay on the market well past the July 13 deadline.
Children 13–15-years old are using e-cigarettes at rates higher than adults in all WHO regions. In Canada, the rates of e-cigarette use among 16–19-year-olds has doubled between 2017–2022, and in England (the United Kingdom) the number of young users has tripled in the past three years. There’s no evidence so far that vaping causes harm to other people around you. If you suspect you have experienced a health-related side effect from using your e-cigarette or would like to report a product defect, report these via the Yellow Card Scheme.
Those primarily using menthol before the ban were both less likely to quit and less likely to continue the same flavor after the ban than those primarily using non-TM flavors before the ban. Those primarily using non-flavored e-cigarettes before the ban were more likely to quit than those primarily used tobacco flavor before the ban. People who used e-cigarettes daily before the ban were less likely to quit and more likely to keep using non-TM flavors.
Never smokers and former smokers were more likely to quit e-cigarettes. While there were no significant differences in quitting all e-cigarette use after each of the three state bans, e-cigarette users in New York were less likely to use banned flavors after the ban than those in New Jersey. The use of menthol flavor was also higher among smokers than former and never smokers. The percent of non-TM flavors had a reverse pattern, being lowest among daily smokers (46.7%), higher progressively along with those smoked weekly or less, former smokers, and highest among never smokers (79.1%). After the ban, the percentages of e-cigarette users who primarily used tobacco, menthol, and non-TM flavors decreased generally except the menthol share among never smokers did not change. After the ban, the percentage of those who quit using e-cigarettes was highest among never smokers (23.1%) and former smokers (13.5%) compared with 3.2% and 7.8% among those who smoked daily and weekly, respectively.

According to the 2021 National Youth Tobacco Survey, more than 2 million U.S. middle and high school students reported using e-cigarettes in 2021, with more than 8 in 10 of those youth using flavored e-cigarettes. Although they’ve been promoted as an aid to help you quit smoking, e-cigarettes have not received Food and Drug Administration approval as smoking cessation devices. A recent study found that most people who intended to use e-cigarettes to kick the nicotine habit ended up continuing to use traditional and e-cigarettes. Conclusion and Relevance These findings suggest that e-cigarette use remained common during the COVID-19 pandemic, particularly among young adults aged 18 to 24 years (18.3% prevalence). Notably, 71.5% of individuals aged 18 to 20 years who reported current e-cigarette use had never used combustible cigarettes.

Our additional analyses (not reported here) showed that a significant difference in compliance exists both between states and between neighborhoods. Respondents living in Washington perceived higher compliance than respondents living in New York and New Jersey, and respondents living in suburban areas perceived higher retailer compliance than respondents living in urban and rural areas. Further studies will be needed to examine the variation of compliance across different geographic areas. He notes that the most effective methods involve some sort of pharmaceutical, either nicotine replacement patches or pills, combined with a program that helps you deal with the problems of quitting smoking. (The American Lung Association recommends Freedom From Smoking.) Even then the average quit rate is just 25 percent, so even the best methods only have a one in four chance of succeeding.

Adverse reactions were infrequent and included primarily throat and mouth irritation in the EC and NRT arms and nausea in the varenicline arm. No serious adverse events were reported in any of the 3 study arms (Table 4). At the 3-month visit, participants were told that they could continue to use their products as needed, but would have to purchase them themselves.

We can also do physical inspections of their manufacturing facilities, or remote records inspections, to verify the information in an application. We were prepared both operationally and from a scientific review standpoint.
In 2014, 50% of Alaska Native adults and 21% of non-Native adults used tobacco products, the report said. By 2021, the rate for Native adults fell to 43% while the rage for non-Native adults was unchanged over that period. While a quarter of all Alaska adults reported that they used tobacco products in 2021, different patterns are emerging according to socioeconomic status, geography and other factors, the new report said. To be authorized, a product’s manufacturer must show it will benefit the health of the population as a whole.
It now appears that e-cigarettes may present their own unique health risks, including to the respiratory and cardiovascular systems. Given the products’ relatively recent introduction to the marketplace, further research is needed to evaluate the short and long term health impacts of e-cigarettes. The harms of e-cigarette use in young people include not only the deleterious effects of nicotine, but also exposure of the lungs and airways to potentially toxic solvents and flavoring chemicals.
One of the most popular e-cigarettes is Juul, but hundreds of different e-cigarette brands are currently available to consumers. In June 2022, the FDA banned Juul from selling it’s products after finding the company provided “insufficient and conflicting data” on risks. In light of this spike in youth e-cigarette use, along with the recent U.S. outbreak linked to more than 2,000 lung illnesses and over 40 deaths, the AMA has called for a total ban on all e-cigarette and vaping products that do not meet FDA approval as cessation tools.

Make sure you buy your vaping products from a reputable retailer so you can be confident they are covered by UK safety and quality regulations.
